    Fill out a quick online form to let us know you are interested.

    Select How to Participate

    Choose to either receive the results only or to directly order the test for your patients.

    Confirm Your Information

    We’ll confirm your information and get you set up in the ordering portal, if needed.

    Refer Your Patients

    Share the program with your patients so they can sign up and participate.

  • 5

    Receive Patients’ Results

    Receive your patients’ results securely. You and your patient will have the option to consult with a genetic counselor to review and discuss the results.
